Psychiatry and the Right to Choose
NHS patients have a legal right to choose their first outpatient appointment. Some GPs aren't sure how they work. Siobhan Rourke found this out when she tried to get an ADHD assessment through Right To Choose.

Right to Choose is the legal right that NHS England gives to patients to choose their own team of doctors to provide non-urgent care led by consultants. This includes psychiatry. This does not apply to services commissioned by local authorities.
Referrals from your GP are required for the right to choose service. This is a GP that is registered with the General Medical Council (GMC). You can find out which GPs are registered with the GMC by looking up their details on the NHS website.
You can request an appointment with Psychiatry UK once you have an established physician. You will receive an email that contains an online link to make your appointment. The link will take you to a secure, encrypted online system where you can make an appointment with a doctor. You'll have to enter some basic information, such as your contact information and GP reference number. Psychiatry UK has a wide range of specialists who can carry out assessments. They include psychotherapists, psychiatrists and nurses. Many of them specialize in a particular condition, such depression or anxiety disorders. However, they may also request to be referred to a different provider if they have an open electronic referral with their GP and the doctor who refers them to it. If you are referred by your doctor to an NHS hospital or service for mental health you have the right to select the location you want to go. This right to choose is applicable only to the services that have an NHS standard contract with your CCG or NHS England for the services you require. This includes psychiatry.
The field of psychiatry studies the causes as well as the treatment and diagnosis of mental illness. It involves the use medication and other methods such as psychotherapy. To better understand how GPs decide on whom to refer to, the TAG team conducted a series of focus groups with doctors and MHPs in eight different GP surgeries. The conversations were recorded and then transcribed exactly. In general, doctors were not inclined to refer patients for therapy or medication. They were worried that the patients wouldn't be able to handle the additional treatment, and that they would not be able to attend subsequent appointments. Additionally, many GPs believed that liaison psychiatry was not being adequately supported and oversubscribed.
The findings of this study revealed that doctors rely more on relationships with patients and knowledge to make referral decisions rather than on the particular needs of a patient. This was evident in all the meetings recorded. CMHT leaders, Psychiatrists, and GPs all spoke about the competence of GPs. This was evident in every meeting taped.
It is important to understand that the right to choose is applicable to treatments as well as assessments, such as neurodevelopmental diagnostic assessments. The current pandemic is causing an enormous increase in the number of people who are being referred to mental health professionals within communities to seek help. This has led to an increase in need for psychiatry assessments and increased workload for staff. The rapid spread of the pandemic meant that it was difficult for GPs and other healthcare professionals to handle the volume of referrals. This led to a huge number of unsuitable referrals.
In response, we conducted a qualitative study nested within an uncontrolled trial that was randomised to explore the effectiveness of a variety of strategies designed to improve the quality of mental health referrals. Interviews with GPs and community mental health team consultants (CMHT) were conducted, as was the recording of meetings to aid in the allocation of referrals. The study found that a custom-designed method for referrals was an effective method to manage the workloads of doctors and CMHTs while giving patients with the option to receive treatment effectively.
